Your rights as a patient

We hope you will find that we provide high quality dental care and that we take time to understand your needs.
We will always ensure your treatment is done in complete confidence by properly trained staff, and that you are involved in decisions about your care.
(the ”care provided” will be any that our dental provides undertake and that which the patient is willing to undergo)

Making appointments

First visit.
Please arrive ten minutes early.

When you attend for your initial consultation, we ask to complete a medical history form, prior to going in the surgery, the Dentist will then discuss any relevant information on the form with you. So, you will need a list of any medication taken, doctors’ details.

Forms are stored electronically. Your link is sent out every visit so you can check your personal sensitive data is up-to date and shared correctly.

Medical History forms are sent out every 9 months, personal sensitive information can be updates by the patient.

If you are an NHS patient, an NHS declaration form must be filled in for every course of treatment. This states if you are exempt from payments or not.

We do not use Direct access to Dental Hygienists in our practice. This means, you must have an examination by the dentist, prior to seeing any of our Hygienists.

Usually, we would ask patients to book their next examination appointment, after all their treatment has been completed. This would be when the dentist recommends, this maybe, 6,12 or even 18 months; appointments are tailored to patient’s needs.

Cancellation of appointments

If you can not attend your pre booked appointment, please give as much notice as possible. If the cancellation is within 48 hours of the appointment, you may be charged a cancellation fee if we cannot refill the slot.

Practice registration

We regard you as a “member” of the practice if you visit the practice for an examination no longer than an 18 month period. There is no official registration for private or NHS patients. Registration for NHS patients was removed by the government in 2004. If you have not been seen within this 18 month period, you may still be seen at the practice as an independent patient (we are not no longer accepting additional NHS patients.) by the clinician who is accepting new/ re registering patients. Your “registration fee” is paid in advance and includes your next exam and relative x-rays (where appropriate).

Payments

We accept Cash, Cards (except American Express/ Amex) and cheques. If you wish to pay by Bacs, this can also be arranged.
We ask patients to pay their accounts on the day of treatment provided.
